I took level 3 in april and had this question on my test, i cannot find anything online or through osha that states a set time period. i believe the test had answer choices of - every year, every 2 years, every 3 years, every 4 years.
Any insight would be greatly appreciated

Only thing I can find is what our safety office refers to as the reason we are required annual training:
How often do I need to review my lockout/tagout procedures?
Employees are required to review their procedures at least once a year to ensure that they provide adequate worker protection. As part of the review, employers must correct any deviations and inadequacies identified in the energy-control procedure or its application.

It's not OSHA requirement for training but a requirement to keep personnel up to date on procedural changes.

NFPA 70E Article 110.2(B)(3) specifies a few separate conditions for LOTO retraining:
Retraining in the lockout/tagout procedures shall be performed as follows:
(1) When the procedures are revised
(2) At intervals not to exceed 3 years
(3) When supervision or annual inspections indicate that the employee is not complying with the lockout/tagout procedures

If the Neta states... every year, every two years, every three years,...?
Would the answer be ... Every 3 years? or annual?
