This question needs more information or the possible answers need to be changed. This schematic could refer to either a form C or D contact setup as the difference between those is in operating sequence, not in form.

The various "forms" are described and laid out in NEMA ICS 5. "Form D" is non existent according to NEMA. IEEE C57.2 also doesn't call out a D, or even a C for that matter. Sometimes you just have to pick the best one.

This website is a great resource but there is some miss information. Form D contacts are used and available from various suppliers. If the question is for NEMA forms only. Asking ”what NEMA form contact is this” would be less confusing.

Thanks for the feedback. Form D will be replaced to help eliminate confusion in the future.
