Has anyone seen an oil sample port that looks like this? If so, what's the best way to extract a sample?

Make sure the main valve is closed and use a pipe wrench to remove that outer nipple and thread an oil sample adapter onto the main valve.

I would shut the valve, and use 1/2" connector with multiple reducers to replace that sample port with my own reducers to take the sample. After your done, re-install the original sample port.
I personally never seen that oil sample connector before, but I always keep 1/2" connector with multiple reducers to a 1/4" adapter for taking a sample off any transformer I come across.
